HORN PUFF Infant T-piece Resuscitators

Heated and humidified T-piece resuscitation is a method of delivering warm, humidified gas to an infant during resuscitation at birth. The T-piece Resuscitator has been used as a primary resuscitation device with newborns. The Infant T-piece Resuscitator or Emergency Resuscitator is used safely to inflate the lungs of infants weighing less than 10 kg to survive the immediate post-natal period using Peak Inspiratory Pressure (PIP) and Positive End Expiratory Pressure (PEEP) controls manually with the use of a test lung and mask. This technique can be performed by healthcare personnel trained in handling resuscitation equipment in emergency care.

Mechanism:
 
The T-piece device is a manually delivered preset, more controlled and consistent set peak inspiratory pressure (PIP) and minimally consistent set positive end expiratory pressure (PEEP) which helps to protect the newborn’s lungs from injury. The goal of T-piece resuscitation is to establish effective spontaneous breathing or assist ventilation of the lungs within the first few minutes of life. The T-shaped tubing is connected to an endotracheal tube and is used to deliver oxygen therapy to an intubated patient who does not require mechanical ventilation. The T-Piece can connect to neonatal masks or endotracheal tubes and helps to establish and maintain functional residual capacity (FRC) by providing a consistent PEEP throughout the resuscitation process, preventing the collapse of alveoli at the end of expiration therapy.

TPR can deliver oxygen levels ranging from 21% through 100%, from a flow meter or blender.

The resuscitator has a PIP control, as well as a maximum pressure relief valve that prevents the accidental delivery of excessively high pressures.

TPR is an accurate, easy-to-read, medical quality manometer, and it can be mounted on a pole, rail, wall, infant warmer, or incubator. TPRs can be reliable and consistently used by healthcare professionals provided the size, fit, and hold of the mask are appropriate.

The Benefits

TPR provides optimum oxygenation by delivering consistent PIP with each breath, avoiding the risks associated with under or over inflation at uncontrolled pressures. The traditional T-Piece Resuscitator design uses a high resistance expiratory valve and a dual flow ratio valve (fluidic flip) to produce PEEP or CPAP during resuscitation. 

Advantages:

Heating and humidifying inspired gases for early stabilisation of preterm infants results in a lower rate of hypothermia and can increase the higher rate of normothermia when compared to the use of cold and dry gases on admission to the neonatal intensive care unit (NICU).
